Parish Registration Form
If you are new to the parish and haven’t yet completed a registration form please do so at the earliest opportunity and drop it in to the parish office. All information is kept confidential and will only be used for parish business

Information Sharing Consent Form
Due to the new GDPR legislation parishes are now required to have written permission to display contact information of activity coordinators. If you coordinate any of the parish ministries and have no objection to your contact details being made known in anyway, written, electronic or verbal, related to parish business please print this form, complete it and return it to the parish office.

Parish Standing Order Form
The Parish depends upon the generosity of its parishioners to maintain the Catholic mission in our area. Use this form to set up a standing order with your bank to ensure your donation to the Church cannot be forgotten. If you are a UK taxpayer please also consider completing a Gift Aid declaration (see below).

Request for Offertory Envelopes
If you would like to make your offertory donations using offertory envelopes please complete this form and hand it in to the Parish Office. If you are a UK taxpayer please also consider completing a Gift Aid declaration (see below).

Parish Gift Aid Declaration
If you are a UK taxpayer and you give to the Parish on a regular basis by standing order or by using offertory envelopes why not sign a Gift Aid declaration? Doing so enables the Parish to claim back the tax you have paid on your donation meaning that currently, for every £1 you donate the Parish actually receives £1.25

Parish Bonanza Application Form
The Parish runs a Monthly Bonanza Draw periodically. If you would like to join the draw please complete this application form and hand it in to the Parish Office complete with your cash or cheque for the correct time period. The Bonanza usually starts in March.
